You wish to send a request to access some public document or information. You do not know how to do that? We have prepared simple manual for you.
I Step... Before you start writing the request, you must clearly know two things: 1) Exactly which information you want to ask for – it is desirable to know in what form that information exist (whether that document is in written, printed, audio, electronic or maybe video format) or eventually some other information that might make this searching easier. 2) What way you wish to access requested information - access to requested information may be realized: a) By a direct insight or rewriting information in premises of Governmental organs b) By rewriting, copying or translating information by Governmental organs. II step... After that you may start writing the request. 1) First, the request should be entitled at organ that you are asking information from. 2) Than, proceed with writing data about requested information. 3) Than, write data about way you would like to access information. 4) At the end, your contact information – name and surname, seat or residence. Depending on the way you choose access and communication of information, you may also leave e-mail address, fax or telephone number. After it has been correctly fulfilled, the request is being submitted to organ that requested information is asked from. The request may be: 1) Submitted personally in archive of Governmental organ (it is obligatory to take 2 examples of request, so your copy is certified too ), 2) Send it by ordinary mail (if possible, registered letter mail) 3) Send by e-mail (*less reliable way of sending the request) 4) Send by fax (*less reliable way of sending the request) Information about seat of Governmental organ, authorized persons for handling requests, information in possession of Governmental organ, may be found in their guides. If in legally defined period of 8 or 15 days (if finding the information is not easy to find or needs more time) you do not receive reply to your request or you are not satisfied with received reply, submit appeal to second degree organ or to complain Administrative Court if it concerns organ whose decision is final and appeal is not allowed.
